4 Your ICI treatment process at Cryos Clinic Planning of treatment At Cryos Clinic, you are involved in planning the treatment, to ensure that it is tailored to your life. Step 1: Fill out the contact form The first step in the process is to fill out our contact form to request an appointment for an initial consultation. You can find the contact request form on cryosclinic.com. Step 1: Submit “Medical History Form” Once you have completed the contact request form, we will send you an email with a link to our Medical History From, which will help determine whether ICI-D treatment is the right choice for you. It is necessary to be thorough when completing the form, as our healthcare professionals will plan the treatment course based on the information. Once we have received the information, we will contact you to schedule your initial consultation. The consultation can take place in the clinic or online. Step 3: Initial consultation During the initial consultation, a healthcare professional from Cryos Clinic will review the information from the Medical History Form with you and present the different options you have going forward. The consultation can take place in the clinic or online. We recommend that you have a normal pap-smear and are tested for chlamydia, HIV, and hepatitis before the consultation. Usually, the initial consultation will result in one of three scenarios: • Scheduling your ICI treatment: If everything is in place, we can schedule your ICI-D treatment right away. The healthcare professional will then inform you about your treatment. All information is given both orally and in writing before your treatment at Cryos Clinic begins. • Need for clarification: If there are any uncertainties or aspects that require further explanation or specification before proceeding with the ICI-D treatment, the healthcare professional will address them with you. • Recommendation of alternative course of treatment: If ICI with donor sperm is not the recommended course of treatment, our healthcare professional will advise you on other treatment options.
